1. Read the following steps before taking action.


2. Use either of the links shown on the right to download the PDF File to your smartphone's memory, unless you have already done so.
   


3. Locate the downloaded file in My Files/Downloads and open it using the Adobe Fill & Sign app. You will probably need to pinch zoom the display to increase the size and scroll around to access the various fields.

4. You should now be able to enter your details into the form by positioning your cursor immediately following each printed heading.
   A selection menu and keypad will appear, for example:
        
5. Unfortunately there is no Tab key to jump to the next item. Instead you must re-position the cursor manually.


6. When you get to the check boxes, simply tap the three dots and an alternate menu will appear. Select ✔ or X, for example:


7. If you make a mistake when in check box mode, select the Trashcan from the selection menu.


8. The Adobe Fill & Sign app has a facility to create a signature and/or initials, both of which are accessible from the pen icon at the top of the app screen. Once you have created a signature, position the cursor over the "Signed" box and then tap the signature that is displayed after you tap the pen icon.


9. In most instances your signature will be too large, but you can adjust the size by dragging the double headed arrows that appear at the right side of your signature and/or dragging the entire image if necessary.


10. When finished, save your completed document to your smartphone.
